REGISTERED NURSE – INFECTIOUS DISEASE
SITE OF SERVICE:
· Fort Bliss – El Paso, TX
POSITION QUALIFICATION/REQUIREMENTS:
· Education: Associates Degree of Nursing.
· Accreditation: Graduate from a college or university accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
· Experience: One (1) year of experience as a registered nurse
· Licensure: Current, full, active, and unrestricted license to practice as a registered nurse in any one of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
· Cardiac Certifications: Must have the following hands-on certification through the American Heart Association or American Red Cross
Basic Life Support (BLS)
· U.S. Citizenship: Shall be a U.S. citizen
· Language Requirement: Shall be able to read, write, speak and understand English well enough to effectively communicate with all patients and other health care providers.
Bilingual preferred
· Physical Capability: Shall be physically capable of standing and/or sitting for extended periods of time and physically capable of performing all services required under the contract.
SPECIFIC DUTIES:
· Collaborate in the design and implementation of initiatives to enhance hospital staff engagement in Medical Management programs.
· Work with Case Management, Utilization Management, Referral Management, and other healthcare teams to ensure seamless, coordinated patient care.
· Deliver services in compliance with MTF policies and protocols.
· Provide comprehensive disease management services, including risk identification, use of clinical guidelines, collaborative care models, patient education, outcome measurement, and routine reporting.
· Analyze population health data to identify high-risk groups, care variation, and inform the development of clinical pathways and practice guidelines; benchmark outcomes against national standards.
· Contribute to and oversee a disease management quality improvement program, recommending corrective actions as needed.
· Develop and implement communication and care coordination strategies for patients with chronic conditions.
· Support preventive care and disease management through evidence-based practices and patient empowerment.
· Continuously evaluate clinical, humanistic, and economic outcomes to improve individual and population health.
· Coordinate care with multidisciplinary teams to address both medical and psychosocial needs.
· Provide nursing consultations in person and by phone, alert physicians to critical patient changes, and ensure proper patient education.
· Support physician-patient relationships and adherence to treatment plans.
GENERAL DUTIES:
· Coordinate patient care in collaboration with a wide array of healthcare professionals.
· Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.
· Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.
· Perform physical exam and health histories.
· Provide health promotions, counseling, and education.
· Administer medications, wound care, and numerous other personalized interventions.
· Direct and supervise care provide by other healthcare professionals.
· Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.
· Recognize adverse signs and symptoms and quickly react in emergency situations.
· Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
· Make referral appointments and arrange specialty care as appropriate.
· Perform nursing services identified in the TO.
· Conduct research in support of improved practice and patient outcomes.
HOURS:
- 8 hour shifts.
- Shifts will be set by the scheduling officer as soon as possible.
SPECIAL REQUIREMENTS/SKILLS:
Must be comfortable in a fast-paced, dynamic environment. Must be able and willing to reprioritize on short notice and work on multiple simultaneous projects. Flexible and able to work with various personalities. Team work skills required. Time management skills required. The ability to meet deadlines in a deadline intensive environment is essential. High level of adaptability and willingness to embrace change in a fast-paced, demanding environment.
