PHARMACIST
SITE OF SERVICE:
· U.S. Naval Hospital Guam
· Full Time
MINIMUM QUALIFICATIONS
· Degree: Bachelor's degree in pharmacy.
· Education: Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
· Experience: Possess a minimum of 3 years’ experience in outpatient and inpatient pharmacy to include ICU, Med/Surg, NICU, Pediatric, and sterile compounding.
· Licensure: Current, full, active, and unrestricted license to practice as a pharmacist.
· U.S. Citizenship: Shall be a U.S. citizen
· English Language Requirement: Shall be able to read, write, speak and understand English well enough to effectively communicate with all patients and other health care providers.
· Physical Capability: Shall be physically capable of standing and/or sitting for extended periods of time and physically capable of performing all services required under the contract.
SPECIFIC DUTIES:
· The HCW shall provide services within the scope of their competencies and the applicable duties provided in the basic contract as well as the following:
· The HCWs shall provide services within the scope of clinical privileges granted by the Department Head, and applicable duties provided in the basic contract.
· Formulate and carry out a goal directed plan of care which based on determined nursing diagnosis and patient outcomes, and which is prioritized according to patient needs and available resources including time, personnel, equipment, and supplies.
· Evaluate effectiveness of patient self-care and ensure all aspects of clinical care support the progression of patient toward set outcomes.
· Perform assessment/data collection in an ongoing and systematic manner, focusing on physiological, and cognitive status.
· Provide a safe and clean environment for each patient.
· Identify patient/significant other learning needs and implement appropriate measures to meet identified needs.
· Assist in planning to LPNs/LVNs, nursing assistants and ancillary personnel.
· Demonstrate sound knowledge base and action in the care and decision making for designated patient situations. If weaknesses or deficiencies are identified either by peer reviews, quality control retorts (QCRs), or evaluations the HCW will be required to complete refresher training (in-services, competency reviews, continuing education, etc.) at Contractor expense.
· Perform documentation (manually and/or by use of computer) duties on unit and in-patient record that are timely, accurate, and concise.
· Participate actively in staff development for unit and Directorate personnel.
· Seek validation of knowledge base, skill level, and decision making as necessary and assertively seek guidance in areas of question.
· Implement care in a knowledgeable, skillful, consistent, and continuous manner.
· Establish priorities of patient care based on essential patient needs and available unit, MTF, and community resources including time, personnel, equipment, and supplies.
· Demonstrate competency in medication and intravenous (IV) administration to include blood products and Total Parental Nutrition (TPN) intramuscular (IM) and subcutaneous (SQ) administration.
· Safely prepares prescribed medications to include intravenous drugs.
· Identify patient/significant other learning needs and implement appropriate measures to meet identified needs.
· Perform documentation (manually and/or by use of computer) duties on unit and in-patient record which are timely, accurate, and concise.
· Participate actively in multifaceted staff development activities at the unit, Director, and MTF level such as clinically mentoring to paraprofessional personnel. Assist the facilitator of training sessions such as MTF’s Basic Cardiac Life Support classes and other training that promotes staff competency and efficiency in emergent situations, and by participating in cross-training initiatives.
· Demonstrate self-directed learning.
· Demonstrate effective and professional communication methods, and skills, using lines of authority appropriately.
· Formulate and use effective working relationships with all health care team members, patients, and significant others. Refer unsolved complaints or infractions to the Division Head with recommendations for appropriate action.
· Practice effective problem identification and resolution skills as a method of sound decision making.
· Remain flexible in staffing patterns and resolution of staffing conflicts; participate in temporary assignment measures.
· Provide individual, group, and family/significant other counseling and health teaching in relation to the patient’s condition and his/her ability for self-care. Provide accessibility to various literature resources dealing with subjects related to specific patient health problems. Provide physical and psychological support to patients and significant others, explain procedures and treatments, and promote cooperation among staff, patients, and significant others.
· Participate in the orientation and training of newly assigned unit personnel. Participate in cross-training initiatives.
· Provide input into competency assessments of newly assigned unit personnel.
· Collaborate in the review of clinic standing operating procedures.
· If directed, perform triage based on protocols and assist as needed in accessing appropriate ambulatory/emergency care for beneficiaries.
· The HCW shall assist during the performance of clinical procedures. Such direction and interaction will adhere to Government and professional clinical standards and clinical protocols.
DUTIES:
· Perform a full range of pharmacist procedures.
· Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility.
· Counsel patients regarding appropriate use of medications.
· Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
· Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for
noted discrepancy of above listed items.
· Compound medications as required.
· Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
· Supervise inventory control of all pharmaceuticals in the pharmacy, especially scheduled
· pharmaceuticals, and ensure pharmacy security. Maintain appropriate records and security of all scheduled controlled substances according to applicable regulations.
HOURS:
Services shall be provided Sunday through Saturday, between the hours of 0000 and 2359 hours. Shifts will normally be scheduled for a six 12.5-hour period and one 8.5-hour period, to include an uncompensated 30-minute lunch break. The Government does not anticipate scheduling each HCW more than 5 Federal holidays a year. For weekend service, the Government may schedule the HCW to work a minimum of two weekends per month (Saturday and/or Sunday). At the mutual agreement of the Contractor and the Government, alternative scheduled may be implemented such as a compressed work schedule. In no instance will the HCW be required to provide services in excess of 80 hours per two-week period.
Base Bown Days:
During a planned closure of the facility due to training, closures before or after holidays family down days, Employee will only be compensated for the actual hours worked. If clinic is closed and employee is not able to work, employee will be required to use leave or take LWOP (without penalty).
SPECIAL REQUIREMENTS/SKILLS
Must be comfortable in a fast-paced, dynamic environment. Must be able and willing to reprioritize on short notice and work on multiple simultaneous projects. Flexible and able to work with various personalities. Team work skills required. Time management skills required. The ability to meet deadlines in a deadline intensive environment is essential. High level of adaptability and willingness to embrace change in a fast-paced, demanding environment.
