SITE OF SERVICE:

·       Walter Reed National Military Medical Center, Bethesda, MD and DiLorenzo Pentagon Health Clinic (DPHC), Washington, DC.

·       HCWs shall receive notification two weeks prior to the reassignment to another location with a 40-mile commuting radius of their assigned MTF.  

 

POSITION QUALIFICATION/REQUIREMENTS:

·       Degree/Education: Physician will have successfully completed an Undergraduate Program, Medical School (M.D. or D.O.), Internship, Residency, and Fellowship Program.

·       Internship/Residency:

o  Successful completion of an internship and residency program (corresponding to the specialty required) which has been approved by the Accreditation Council for Graduate Medical Education or the Committee on Postdoctoral Training of the American Osteopathic Association.

o  Subsequent to obtaining a Doctor of Medicine or Doctor of Osteopathy degree, a candidate must have had at least 1 year of supervised experience providing direct service in a clinical setting (i.e., a 1-year internship or the first year of a residency program in a hospital or an institution accredited for such training).

o  For purposes of this requirement, graduate training programs include only those internship, residency, and fellowship programs that are approved by accrediting bodies recognized within the United States or Canada. Descriptions of such programs are described below.

o  An internship program involves broadly based clinical practice in which physicians acquire experience in treating a variety of medical problems under supervision (e.g., internal medicine, surgery, general practice, obstetrics gynecology, and pediatrics). Such programs are in hospitals or other institutions accredited for internship training an appropriate accrediting body.

o  A residency program involves training in a specialized field of medicine in a

hospital or an institution accredited for training in the specialty by an appropriate

accrediting body.

o  A fellowship program involves advanced training (beyond residency training) in a

given medical specialty in either a clinical or research setting in a hospital or an

institution accredited in the United States for such training.

·       Board Certification: Board eligible or board certified as required.

·       Additional Certifications: Must have the following hands-on certification through the American Heart Association or American Red Cross: Current Basic Life Support (BLS)

·       Experience: Minimum of one (1) year experience within preceding three (3) years in the specialty of Allergy and Immunology.

·       Licensure: Current, full, active, and unrestricted license to practice medicine.

·       U.S. Citizenship: Shall be a U.S. citizen

·       English Language Requirement: Shall be able to read, write, speak and understand English well enough to effectively communicate with all patients and other health care providers.

·       Physical Capability: Shall be physically capable of standing and/or sitting for extended periods of time and physically capable of performing all services required under the contract.

Additional Requirements:

·       Shall be knowledgeable of equipment and supply terminology and skill sufficient to identify and use necessary equipment and supplies properly and to communicate proper use with nurses, medical staff, patients, and family members.

·       Shall be sufficiently skilled to teach patients proper self-care and to motivate patients and family members to continue the procedures for promoting rapid healing.

·       Shall be knowledgeable and possess skills necessary to provide care appropriate to any agerelated needs of the patients. The HCW shall demonstrate knowledge of the principles of growth and development appropriate to the appropriate patient population served: Infants, Children, Adolescent, Adults, and Geriatric (>65).

·       The HCW Allergist must demonstrate skills in interviewing, examination, assessment, and management of patients with general medical and surgical health problems. Seriously ill patients shall be managed in consultation with or by direct referral to specialty physicians.

DUTIES:

·       Provide care for acute allergic problems such as status anaphylaxis and allergic reactions.

·       Provide consultative support for Tri-service medical treatment facilities within the US and foreign countries.

·       Aid Health Service in organization of mass vaccination programs. Direct preparation and administration of vaccines used in treatment and diagnostics of allergic disorders.

·       Perform tasks that include, but are not limited to, the following: skin testing, rush desensitization, spirometry interpretation, food and medication challenge, and smallpox immunization.

·       Protect patient health information from unauthorized disclosure in accordance with Health Insurance Portability and Accountability Act (HIPAA) requirements.

·       Participate in monitoring quality of care according to professional standards and established procedures.

·       Examine all eligible military health care system beneficiaries presenting themselves for medical care at the outpatient specialty clinic and/or inpatient setting, to render a medical assessment of their conditions and to take appropriate action to include referrals where medically indicated.

·       Prepare appropriate narrative summaries and patient profile changes. The contract provider shall fully document all medical information in the patient record. This documentation shall be sufficient to provide any other provider seeing the patient with the necessary information on prescribed procedures, medication follow ups, and changes in condition.

·       Prepare and document appropriate history, physical examination, and preoperative diagnosis, to include physical limitations.

·       The HCW will participate in monitoring quality of care according to professional standards and established procedures.

·       The Contactor will maintain medical records IAW AR 40-66 (Medical Record Administration and Healthcare Documentation). Medical records will be subject to review by the Medical Care Evaluation Committee, Quality Assurance Committee, and designated representatives of the WRNMMC Commander.

·       Maintain documentation of all treatment provided IAW WRNMMC directives and prepare such records and reports as may be required by the contract. Ensure all documentation and reports are legible. Submit prepared documents prior to leaving at the end of the shift. Complete Armed Forces Health Longitudinal Technology Application (AHLTA).

·       Refer all requests for patient/clinical information, which cannot be handled by the clinical staff to the Administrative Officer of the Day (AOD) or Patient Administration Division (PAD). The contract employee shall not release medical information without coordination with the AOD or PAD.

·       Prescribe pharmaceuticals according to the availability of drugs. The pharmacy services will provide instructions to all prescribing HCWs on substitutes and generic drugs for prescribed drugs.

·       The quality of Allergy and Immunology physician services shall meet or exceed reasonable standards of professional practice as determined by the authority that governs military medical professionals in the same discipline.

·       The provision of services under this contract is part of the managed care network of providers under the Department of Defense TRICARE Program. The managed care network of military facilities, government contractors, and civilian preferred providers, establishes a continuum of medical services for eligible beneficiaries. As part of the network, the HCW will be required to interface with network participants, including other government contractors in order to appropriately manage services for the beneficiaries. Contact with other government contractors will be for the purpose of coordinating services only. All contract compliance and performance guidance will come from the Contracting Officer and/or the COR.

·       The HCW shall provide patient treatment and consultation to other member of the medical staff. HCW providing services under this contract shall be required to perform their assigned duties with regard to all admitted patients and eligible beneficiaries regardless of underlying disease process or physical condition of such patients or beneficiaries.

·       The HCW shall see approximately five to six (5-6) new patients per day, 45-minute appointments, and approximately nine to ten (9-10) follow-up, 20-minute appointments per day.

·       The HCW shall evaluate and manage urgent or emergency problems as such patients present for care. a. Consult with consultants as appropriate. b. Provide medical advice over the telephone. c. Complete all required paperwork. d. Obtain an adequate history, physical assessment, and plan in a timely and appropriate manner on each patient and develop a treatment plan. e. Utilize paraprofessional staff as appropriate. f. Respond to cardiopulmonary arrest as appropriate. g. Make preliminary interpretation of radiographs. h. Perform documentation and patient care in accordance with Joint Commission on Accreditation of Hospitals (JCAHO) guidelines.

·       The HCW shall instruct and monitor other medical personnel in care and treatment of patient to include non-therapeutic services such as physical examination. The HCW shall provide medical supervision and instruction for physician extenders such as nurse practitioners and physician assistants.

·       The HCW shall initiate requests for tests, as appropriate. Follow-up of abnormal reports is the responsibility of the ordering physician. All received reports on patients seen by the HCW shall be reviewed by the HCW within 24 hours of receipt.

·       The HCW shall prepare and document appropriate history, physical examinations, and diagnosis.

·       The HCW shall maintain documentation of all treatment provided in accordance with hospital directives and prepare such records and reports as may be required. The HCW shall ensure all documentation and reports are legible.

·       The HCW shall be capable of performing the full range of services in the diagnosis and treatment of allergy and immunology to include, but not limited, to the following: a. Perform Medical Evaluation Boards IAW with AR 40-501. b. Administration of Immune Therapy. c. Diagnosis and management of Diseases: Asthma, Allergic Rhinitis, Urticate/Angioedema, Food Allergy, Drug Allergy, Chronic Obstructive. Pulmonary Disease (COPD), Immune Deficiency, Recurrent Infections, Hymenoptera Allergy and Anaphylaxis. d. Drug Desensitization. e. Drug Challenge. f. Pulmonary Function Interpretation. g. Epicutaneous Skin Testing. h. Intradermal Skin Testing. i. IVIG Administration. j. Endotracheal Intubation. k. Rhino-laryngoscopy. l. Methacholine Challenge.

CORE DUTIES:

·       Perform a full range of physician services in accordance with privileges granted by the MTF.

·       Direct, perform, or assist in the instruction of other health care professionals within the scope of the clinical privileges or responsibilities.

·       Examine, diagnose, treat or prescribe courses of treatment within the scope of training, experience, and privileges Provide preventive and health maintenance care, including annual physicals, positive health behaviors, and self-care skills through education and counseling.

·       Order diagnostic tests as applicable as well as request consultation or referral with appropriate physicians, clinics, or other health resources as indicated.

·       Technically proficient in directing and teaching other medical staff, providing educational lectures, and participating in the provision of in-service training to clinic staff members. Such direction and interaction will adhere to Government and professional clinical standards and accepted clinical protocols.

HOURS:

·       Walter Reed National Military Medical Center (WRNMMC) and DiLorenzo Pentagon Health Clinic (DPHC): Operates 24 hours daily/7 days weekly/365 yearly/366 days in leap years for inpatient services including holidays.

·       Full-time contractor personnel typically work 40 hours per week unless mission dictates changes.

·       Operates Monday – Friday 7 a.m. to 4 p.m., closed on weekends and federal holidays. Duty hours include a one-hour, unpaid lunch period, excluding federal recognized holidays and other times the clinic is authorized to be closed by the Clinic Director.

·       Duty hours will not exceed 80 hours in a two-week period. Hours worked do not include travel to and from the work location.

·       Due to the nature of patient care, HCWs may be required to work beyond 40 hours per week; however, hours will not exceed 80 hours in a two-week period.

·       Hours exceeding 40 hours per week will be coordinated and adjusted with the Contractor, Government supervisor, and COR to not exceed 80 hours within the current two-week period.

·       HCWs will be paid only for the hours worked at the established rate for the CLINs. Overtime is not authorized.

 

SPECIAL REQUIREMENTS/SKILLS

Must be comfortable in a fast-paced, dynamic environment. Must be able and willing to reprioritize on short notice and work on multiple simultaneous projects. Flexible and able to work with various personalities. Teamwork skills required. Time management skills required. The ability to meet deadlines in a deadline intensive environment is essential. High level of adaptability and willingness to embrace change in a fast-paced, demanding environment.
