REGISTERED NURSE (CASE MANAGER)
SITE OF SERVICE:
Intrepid Spirit Centers at ATAMMC (Fort Belvoir), Camp Lejeune, Camp Pendleton, Fort Campbell, Joint Base Lewis-McChord, and NICoE (Bethesda)
HOURS:
Monday through Friday, 8- or 10-hour shifts between 0600-1800 (typical clinical hours 0700-1600). 40 hours per week. No on-call or Federal holidays required.
POSITION OVERVIEW:
The Registered Nurse Case Manager will provide patient management, care coordination, and discharge/disposition planning for outpatient settings at the Intrepid Spirit Centers under a Defense Health Agency (DHA) personal services contract. The RN Case Manager assists service members, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs related to traumatic brain injury (TBI), combat stress, and psychological health conditions.
POSITION QUALIFICATION/REQUIREMENTS:
• Education: Graduate from a college or university accredited by ACEN, CCNE, or equivalent. BSN preferred. Foreign nursing graduates must have degree equivalency validation.
• Case Management Certification: Must possess one of the following: CCM, CDMS, CRRN, COHN/COHN-S, ACCC, CRC, RN-NCM, or CMC certification.
• Licensure: Current, full, active, and unrestricted registration as a graduate professional nurse in any U.S. State, Territory, Commonwealth, or DC.
• Certifications: Basic Life Support (BLS) certification required.
• Experience: One year of experience in nursing after graduation.
• Citizenship: U.S. Citizen required.
• Security: Must possess and maintain Tier III security investigation.
DUTIES:
• Provide competent, evidence-based care to assigned patients.
• Provide patient management, care coordination, and discharge/disposition planning for outpatient settings.
• Assist family members and caregivers with receiving most appropriate options and services to meet complex health care needs including acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress; residuals of TBI; community adjustment; and addictions.
• Coordinate care with multiple providers across all levels and sites of care.
• Address psychosocial, nursing, and medical needs of patients and families through participation in interdisciplinary patient care management practices.
• Evaluate care and outcomes to ensure timely and appropriate provisions of services.
• Coordinate and implement discharge plans by arranging for transition to the next level of care in compliance with multidisciplinary treatment plan.
• Document outcomes at the program or service level.
• Execute position responsibilities demonstrating leadership, experience, and creative approaches to management of complex patient care.
• Evaluate outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.
• Document and close notes in MHS Genesis within three (3) business days of patient encounters.
• Respond to beneficiary messages in MHS Genesis secure portal within one (1) business day.
SPECIAL REQUIREMENTS/SKILLS:
Must be comfortable working with complex TBI and psychological health cases. Must possess knowledge of Dragon dictation software, MHS Genesis, and Microsoft Office programs. Must comply with regulatory, MTF, and Joint Commission quality inspection/peer review/utilization management requirements. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility.
