SITE OF SERVICE:

 

·       Provide Registered Nurse services in support of the Medical Home Port (MHP) Primary Care Clinics at the Naval Hospital Jacksonville, FL. The objective of this program is to enhance continuity of care, wellness, and disease management for patients.

·       The health care worker (HCW) will be assigned to a primary location; however, the Government retains the right to assign to another location within a fifty (50) mile commuting radius of their assigned Military Treatment Facility (MTF). Shall be notified two weeks in advance prior to reassignment to locations within a fifty-mile limit.

POSITION QUALIFICATION/REQUIREMENTS:

·       Degree: Possess Associates Degree of Nursing

·       Education: Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE).

·       Experience: One year of experience in nursing after graduation

·       Certifications:

o  Possess and maintain current Basic Life Support (BLS)

o  Advanced Cardiac Life Support (ACLS)

o  Pediatric Advanced Life Support (PALS) Certifications

·        Licensure: Possess and maintain a current unrestricted license to practice as a registered nurse in any one (1) of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.

·       CME Requirement: Have successfully completed at least twelve (12) classroom hours of continuing education within the preceding twenty four (24) months that maintains skills and knowledge as an RN.

·       U.S. Citizenship: HCWs performing under this contract shall be U.S. citizens.

·       Able to read, write, and speak English well enough to effectively communicate.

·       Shall be physically capable of standing and/or sitting for extended periods of time and physically capable of performing all services required.

REFERENCES:

·       Provide three (3) letters of recommendation from practicing physicians or peers attesting to his/her clinical skills.

·       Reference letters must include name, title, phone number, date of reference, address and signature of the individual providing reference.

·       Letters of reference must have been written within the preceding two (2) years.

SPECIFIC DUTIES:

·       Perform a full range of RN duties in accordance with assignment under the task order, including: triage; patient assessment and monitoring; use of patient monitoring and treatment equipment; appropriate nursing care, procedures, and treatments; execution of physicians’ orders within the guidelines of standard nursing practice; documentation of patient care and observations; and patient education and emotional support.

·       Serve as the first line primary care team member for the Medical Home Port team’s patient population and assigned nursing service personnel. The HCW will be an active participant in the primary care team and has a level of responsibility for the oversight and care coordination for the patient’s needs. The HCW will address medical issues in a compassionate, comprehensive and integrative manner utilizing a collaborative approach with emphasis on prevention of disease and promotion of health and wellness. The HCW is responsible for ensuring the standards of ambulatory care nursing practice are met.

·       Administer immunizations and IV infusion medications in accordance with clinic protocols.

·       Administer specific medications at designated times and in designated routes.

·       Chart medication, dosage and time given and indicate if not administrated.

·       Observe patients and report pertinent data on condition and medication or treatments.

·       Chart nursing observations and ensure record is complete with all procedures and test results recorded. Maintain required patient charts, forms, logs, records, registers, and census reports and collect required statistics.

·       Prepare equipment and assist providers and other team members with treatment procedures.

·       Recognize emergencies and respond with appropriate protocols, e.g., oxygen, emergency drugs, Basic Cardiac Life Support (BCLS), ACLS, PALS, application of medical equipment and other, as established by the medical staff. NHJ-N-0022 Naval Hospital Jacksonville, FL

·       Explain procedures and treatments to patients and significant others to allay apprehension, provide patient/family education, and gain cooperation and support.

·       Work directly with patients and families to develop a plan of care to meet needs. Collaborate with other health care disciplines to provide total quality patient care.

·       Function as medical home care coordinator to assure all aspects of patient care needs are met, to include assessment, intervention and follow-up. Assist providers in completing telephone consults as part of comprehensive medical home approach to caring for patients.

·       Obtain and document patient and pertinent family history. Orient new patients and their families as required.

·       Receive patient assignments, formulate, document, and implement a nursing care plan for assigned patients and review and update them as required.

·       Check patient identification, physician’s orders, and preadmission and transfer information as applicable. Requisition and arrange for ordered diagnostic and therapeutic services as directed.

·       Assist in defining patient populations that require professional nursing intervention and the establishment of guidelines to evaluate selected patients.

·       Function as the team nurse for the medical home concept, to include actively assisting patients with preventative health care.

·       Become familiar with and utilize the various computer programs to assist with patient care.

·       Customer Service Accountability – Contract employees providing services at Naval Hospital Jacksonville, FL and supporting facilities shall adhere to the Customer Service standards specified in NAVHOSPJAXINST 6320.7F or updated versions. A copy of the instruction will be made available upon request or at the time of commencement of services.

GENERAL DUTIES:

·       Maintain a level of productivity comparable with that of other individuals performing similar services.

·       Participate in peer review and performance improvement activities.

·       Practice aseptic techniques as necessary. Comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection.

·       Function with an awareness and application of safety procedures

·       Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Anticipate potential problems/emergencies and make appropriate interventions. Notify supervisor, director, or other designated person regarding problems that the HCW is unable to manage.

·       Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.

·       Participate in the implementation of the Family Advocacy Program as directed. Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting.

·       Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.

·       Maintain an awareness of responsibility and accountability for own professional practice.

·       Participate in continuing education to meet own professional growth

·       Attend and/or comply with all annual training classes required by the Command, to include but not limited to online annual training provided by the MTF: disaster training, infection control, Sexual Harassment, Bloodborne Pathogens, Fire and Safety, Chemical, Biological, Radiological, Nuclear and Explosives (CBRNE), and all other required training.

·       Actively participate in the command’s Performance Improvement Program. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist.

·       Participate in the provision of in-service training to clinic staff members. Provide training and/or direction as applicable to supporting government employees (e.g., hospital corpsmen, students, etc.).

·       Attend and participate in various meetings as directed.

·       Perform timely, accurate, and concise documentation of patient care.

·       Operate and manipulate automated systems such as CHCS, AHLTA, ADS, Essentris, MHS Genesis and Clinical Information System (CIS), participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander. Maintain DOD email account as directed. The HCW shall be responsive to all email and voicemail communications.

·       Exercise appropriate clinical delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities.

·       Maintain documentation of all treatment provided in accordance with clinic directives, and prepare such records and reports as may be required. All records and reports must be legible. Abbreviations must be only those listed in local instructions.

CORE DUTIES:

·       Coordinate patient care in collaboration with a wide array of healthcare professionals.

·       Facilitate the achievement of optimal outcomes in relation to clinical care, quality and

cost effectiveness.

·       Ensure compliance with standards of care and practice in accordance with all established

policies, procedures, and guidelines used in the MTF.

·       Perform physical exam and health histories.

·       Provide health promotions, counseling, and education.

·       Administer medications, wound care, and numerous other personalized interventions.

·       Direct and supervise care provide by other healthcare professionals.

·       Accountable for making patient care assignments based on the scope of practice and skill

level of assigned personnel.

·       Recognize adverse signs and symptoms and quickly react in emergency situations.

·       Communicate and collaborate with a diverse group of people for the purpose of

informing the healthcare team of plans/actions, for teaching/education to benefit the

patient/family and organization.

·       Make referral appointments and arrange specialty care as appropriate.

·       Conduct research in support of improved practice and patient outcomes

HOURS:

·       The Primary Care Clinics are open from 0630 until 1900, Monday through Friday.

·       Shift lengths may be variable, 8.5 or 9-hour period (to include an uncompensated 30 to 60 minute meal break, respectively) to ensure shift coverage.

·       In no instance shall the HCW be required to provide services in-excess-of 80 hours per two-week

period.

SPECIAL REQUIREMENTS/SKILLS

Must be comfortable in a fast-paced, dynamic environment. Must be able and willing to reprioritize on short notice and work on multiple simultaneous projects. Flexible and able to work with various personalities. Teamwork skills required. Time management skills required. The ability to meet deadlines in a deadline intensive environment is essential. High level of adaptability and willingness to embrace change in a fast-paced, demanding environment.
