CLINICAL PHARMACIST

SITE OF SERVICE

  • Services provided in support of the Naval Hospital Pensacola. The primary site of services shall be supporting the Navy’s Medical Home Port (MHP) program.
  • Full time.

POSITION QUALIFICATION/REQUIREMENTS:

  • Degree: Doctor of Pharmacy (Pharm.D.).
  • Education: Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
  • Internship/Residency: Completion of a clinical pharmacy residency or fellowship
  • accredited by the American Society of Health System Pharmacists or American College
  • of Clinical Pharmacy or documentation of appropriate education, training, and/or CME in
  • the practice of clinical pharmacy.
  • Experience:  Completion of PGY-1 Residency or one (1) year of experience in medication therapy management within the last 3 years
  • Licensure: Current, full, active, and unrestricted license to practice as a pharmacist.
  • U.S. Citizenship: Shall be a U.S. citizen
  • English Language Requirement: Able to read, write, and speak English well enough to effectively communicate.
  • Physical Capability: Shall be physically capable of standing and/or sitting for extended periods of time and physically capable of performing all services required under the contract.

CORE DUTIES:

  • Assess patient’s response to drug therapy and planning drug therapy based on physician established diagnoses.
  • Order and evaluate laboratory tests necessary to evaluate drug therapy effects and
  • outcomes.
  • Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states
  • (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in
  • cooperation with the medical staff.
  • Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or
  • changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin,
  • antithrombotic).
  • Administer prescription or non-prescription drugs according to established agreements or
  • MTF protocols.
  • Assess metabolic needs and order therapeutic enteral or parenteral nutrition products in
  • the inpatient setting.
  • Evaluate medical and medication histories for drug-related problems and adjusting drug
  • therapy accordingly.
  • Consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical
  • therapists, etc.) about patient treatment needs or options.
  • Perform a full range of pharmacist procedures.

HOURS:

  • Services shall be required in the assigned clinical area Monday through Friday, excluding federal holidays, between the hours of 0730 and 1730 for an 8.5 or 9 hour shift to include an uncompensated 30-minute or 1 hour meal break depending on shift length.
  • In no instance shall the contractor be required to provide services in excess of 80 hours per 2-week period.
  • The specific schedule for each 2-week period will be scheduled 1 month in advance by the government supervisor.

Must be comfortable in a fast-paced, dynamic environment. Must be able and willing to reprioritize on short notice and work on multiple simultaneous projects. Flexible and able to work with various personalities. Teamwork skills required. Time management skills required. The ability to meet deadlines in a deadline intensive environment is essential. High level of adaptability and willingness to embrace change in a fast-paced, demanding environment.
